The gallbladder is a small pear-shaped organ in the upper right quadrant of the abdomen, beneath the liver2. This hollow organ measures approximately 2.7 to 3.9 inches in length and up to 2 inches across at its widest point.
Understanding the Gallbladder’s Anatomy
The gallbladder consists of three main parts: the fundus, body, and neck5. The fundus is the rounded base that faces the abdominal wall, while the body lies in a depression on the liver’s surface. The neck tapers and connects to the cystic duct, which joins the common hepatic duct to form the common bile duct. The network of ducts, known as the biliary system, connects the gallbladder to the liver and small intestine, allowing for the storage and transport of bile6.

Role of the Gallbladder in the Digestive System
The gallbladder's primary function is to store and concentrate bile, a yellowish-brown fluid produced by the liver. Bile plays a vital role in the digestion of fats in our diet. The gallbladder contracts and releases bile into the small intestine through the common bile duct when we consume fatty foods. This bile helps break down large fat globules into smaller droplets, making it easier for digestive enzymes to process and for the intestines to absorb fats.
The Gallbladder and Bile Production
While the liver produces bile, the gallbladder is a storage and concentration facility. The liver produces about 767 to 1005 mL of bile daily2. Between meals, when there’s no immediate need for fat digestion, most of this bile flows into the gallbladder.
Bile is concentrated and stored here until needed. The gallbladder can hold approximately 29.6 to 79.8 mL of concentrated bile7. When we eat fatty food, the gallbladder contracts, releasing this concentrated bile into the small intestine to aid digestion.

The gallbladder’s primary function is to store and concentrate bile produced by the liver. When bile enters the gallbladder, it undergoes a concentration process. The gallbladder removes water and electrolytes, particularly sodium and chloride ions, from the bile8.
This process can concentrate bile three to ten times its original strength, significantly enhancing its digestive capabilities. The concentrated bile is then stored until needed.
Bile Release During Digestion
When we consume fatty foods, the gallbladder gets to work. The presence of fats in the small intestine triggers the release of cholecystokinin (CCK) hormone9. In response to CCK, the gallbladder contracts rhythmically, releasing its stored bile into the common bile duct10. From there, the bile flows into the duodenum, the first part of the small intestine. This timely release of bile is crucial for the efficient digestion of fats, as it coincides with the arrival of fatty food in the small intestine11.
Importance of Bile in Digestive Health
Bile is vital for our digestive health, primarily by aiding in the breakdown and absorption of fats. When bile enters the small intestine, it emulsifies, breaking down large fat molecules12. This process increases the surface area of fats, making them more accessible to digestive enzymes13. As a result, fats are more easily absorbed through the intestinal walls. Additionally, bile serves an important function in waste elimination. It is a vehicle for removing bilirubin, a byproduct of haemoglobin metabolism, from the body14.

Common Gallbladder Issues
Gallstones are the most prevalent gallbladder problem, affecting approximately 20 per cent of the global population15. These hardened deposits can either be cholesterol stones (the most common type) or pigment stones. Another frequent issue is cholecystitis, a gallbladder inflammation often caused by gallstones blocking the bile ducts16.
Symptoms of Gallbladder Problems
Gallbladder problems typically manifest with several key symptoms17:
- Sharp pain in the upper right abdomen, often radiating to the back or right shoulder.
- Sudden and intense abdominal pain lasting 1-5 hours, sometimes triggered by fatty meals.
- Nausea and vomiting.
- Fever and chills.
- Jaundice (yellowing of the skin and eyes)
- Itchy skin
- Light-colored stools and dark urine

- Pancreatitis: Gallstones can block the pancreatic duct, leading to pancreas inflammation21.

Diagnostic Tests and Tools Used To Evaluate Gallbladder Health
Doctors employ various methods to screen for gallbladder problems23:
- Ultrasound: The primary and most accurate tool for detecting gallstones, with a 90 - 95 per cent accuracy rate.
- CT scan: Useful for identifying acute gallbladder disease, polyps, and occasionally gallbladder sludge.
- MRI: Provides detailed images of organs and can reveal gallstones in the biliary tract.
- HIDA scan: Assesses gallbladder function and is particularly useful when ultrasound results are negative24.
- Blood tests: Can indicate infection or inflammation in the gallbladder or surrounding organs17.
In some cases, multiple tests might be necessary to confirm a diagnosis.
Treatment Options for Gallbladder Issues
Treatment for gallbladder problems varies depending on the severity. Reducing fat intake and maintaining a healthy weight can help manage mild symptoms25. Ursodeoxycholic acid might be prescribed to dissolve small gallstones, and cholecystectomy (gallbladder removal) is often recommended for recurrent gallstones or severe inflammation26,27.
Many people lead healthy lives without a gallbladder. After removal, the liver produces bile, which flows directly into the small intestine. While some dietary adjustments might be necessary, most individuals adapt well to life without a gallbladder.
